  • A government issued ID must be included with POA.

POA Instructions

  1. 1.

    Select type of company by marking the appropriate box.

  2. 2.

    Grantor's Employer Identification Number (EIN)/Federal Tax Identification Number in the format XX-XXXXXXX. If the Grantor is an individual, indicate the Social Security Number, in the format XXX-XX-XXXX.

  3. 3.

    Indicate the legal name of the Grantor as registered with the IRS. It must be the full legal name associated with the number listed in step 2.

  4. 4.

    If Grantor is registered with a fictitious name ("doing business as") list the name. If not, leave blank.

  5. 5.

    State in which the Grantor is doing business.

  6. 6.

    List the physical address of the Grantor, including the address, city, state, and zip code. If a foreign Grantor, also include the country. (Must be complete – no P.O. box.)

  7. 7.

    Email Grantor receives electronic correspondence at.

  8. 8.

    State in which the Grantor is doing business.

  9. 9.

    Grantor's name, same as step 3.

  10. 10.

    Signature of an officer or authorized representative of the Grantor.

    1. If the Grantor is a corporation, this step must be signed by the CEO, COO, CFO, President, Vice President, Treasurer, or Corporate Secretary with no variations.
    2. If the Grantor is a partnership, the signor must be a partner. The Grantor must provide a copy of the partnership agreement to confirm the name of the partner(s) authorized to sign the POA on the partnership's behalf.
    3. If the Grantor is an LLC, this step must be signed by a Partner, Managing Member, or an officer such as President, Vice President, Treasurer, Corporate Secretary with no variations.
    4. If the Grantor is a non-resident entity, the non-resident certification is required to confirm the capacity of the individual who signed the POA.
  11. 11.

    Print the name of the signatory.

  12. 12.

    Indicate the title of the signatory (i.e. CEO, COO, CFO, etc as stated in step 10).

  13. 13.

    Indicate the date signed by the signatory.

We must receive attached with POA a copy of the signatory's government issued ID.
